CURATIVE MEDICAL

Is Curative covered in all 50 states?

Yes! Please use the Curative Provider search to find in-network care near you or whatever locations you frequently travel to. 

You will be assigned a designated Care Navigator that can also assist in finding and transferring care to Curative in-network providers during your Baseline visit.

What will my costs be for medical care before I complete my Baseline visit?

You are covered under the $0 plan for the first 120 days. So, if you need a doctor before you can complete your Baseline visit, no problem!

On day 121, if you have not completed your Baseline visit then you will no longer be covered under the $0 plan, and will incur out-of-pocket costs up to your deductible and out-of-pocket max limits. 

What is the Baseline visit? Do all of my dependents have to complete one?

The Baseline visit is a telehealth visit with your personal Care Navigator and a licensed Curative clinician. This visit is separate from your regular, annual wellness visits, and is meant to support you with getting started with your plan, finding in-network care, and getting you the resources for you and your family's individual needs.

Every individual over the age of 18 must complete their own Baseline visit within 120 days of coverage start to maintain their $0 plan. 

If you have dependents under the age of 18, at least one custodian must complete the visit for the minor dependent to get the $0 coverage benefits. 

What is the Curative Cash Card? What does it mean if my provider is "Curative Cash Card Only"?

The Curative Cash Card helps you access $0 care, even when your Member ID Card can’t be used. It’s designed for providers who accept cash payment instead of billing insurance. Use at providers marked as Cash Card on the provider search, or if the provider is on the provider search and won’t take your Curative Member ID Card. You pay nothing—Curative covers the cost directly.

Are labs, X-Rays, MRIs, and other scans covered at $0?

If you complete your Baseline visit in the first 120 days, and the procedures are medically necessary, then yes!

Elective or exploratory procedures may not be covered or may require additional pre-authorization. 

Please speak with your Care Navigator before completing any procedures that are not medically necessary to ensure you will covered or have a full understanding of your financial responsibilities. 

Will I have a gap in medication coverage until I complete my Baseline visit?

No. You will be able to transfer your medications on the first day of coverage, and if they are Tier 1 prescriptions, they will covered 100%.

Completing the Baseline visit in the 120 day window will ensure that you get to keep that $0 cost all year long. 

What happens if I don't complete my Baseline visit in the first 120 days?

If you do not complete your Baseline visit in the 120 day window, you will be subject to the deductibles, copays, and coinsurance limits detailed in the Internal Benefits Guide.
